Below you will find information and a link to a study being conducted by Columbia Universty's Teacher's College, targeting African American women with a child between ages of 8 and 14, related to a new website: "MyPlate Project for Moms and Children".  Once there have been 200 responses,  the study will close. We have been asked to send to child care programs to give you the opportunity to share this information with your staff or parents that fit that criteria.

YOU ARE INVITED TO TAKE A SURVEY.

1.             The Research Group on Disparities in Health within the Department of Health   and Behavior Studies at Teachers College, Columbia University, in New York, New York is studying the impact of The MyPlate Project for Moms and Children, a new website with cartoon-like videos and online activities that will allow you to learn about First Lady Michelle Obama's work introducing MY PLATE (replacing the old Food Pyramid) to promote healthy eating and increase physical activity to combat the nation's obesity epidemic.

2.            You are being asked to fill out a survey to help us learn "what works" to inspire healthy eating and physical activity behaviors in moms and children.

3.          Participation in this survey is limited to the first 200 women who successfully complete it. It takes about 45 minutes to complete the entire study--including spending part of that time watching cartoon videos with your child; you also help your child to rate the videos after watching them.

4.          After 200 people complete the survey, the computer program will automatically close the survey and send out bar-coded gift certificates for use at www.Amazon.com<http://www.amazon.com/> to three randomly chosen e-mail accounts ($300, $200 or $100) without in any way linking your identity to the survey results.

5.          Please go to   MomAndChildPlates<https://tccolumbia.qualtrics.com/SE/?SID=SV_3OiMTYjYg6NsV6Z> view the informed consent, learn about your rights as a participant and proceed to the survey, and watch videos with your child for 18 minutes.
