Dear members of the NYS child care community,

We need your help. The NYS Integrated Pest Management (IPM) Program is administering an online survey to child care providers to learn more about the status of pest management within child care centers. The survey (see attached for a preview) will assess the types pests encountered, level of knowledge of staff, how pest management services are obtained, pesticide use patterns and interest in IPM. The results from this survey will inform our projects and ultimately assist child care providers in protecting children from both pests and pesticides.

If you are a child care provider in New York State, we would greatly appreciate your time in completing this 10-minute survey. If you support child care centers, we would greatly appreciate your time in distributing this survey to child care providers in your area.

The deadline to complete the survey is Wednesday, December 3, 2014.

Participation in this survey is voluntary and your individual responses will be kept confidential. A summary of the findings will be made available to the public.
