date for the immunization survey that NYS DOH requires you to complete. If

Coordinating Council & Child Care Councils

 

Dear Child Care Coordinating Council and Child Care Council Representatives:

We are moving the day care/pre-k survey to the beginning of February with a
March 5 due date. 

The October 31, 2011 survey due date will apply only to all public and
non-public schools (Pre-k/K through 12).

The March 5, 2012 survey due date will apply to all day care centers, Head
Start Programs, nursery schools and group family day cares.

We will be creating a separate online day care survey and are actively
contacting day care centers, Head Start programs and nursery schools
regarding submitting online. 
Group family day cares are welcome to submit online, but we are not actively
contacting them at this time. We anticipate making it mandatory for day care
centers, Head Start programs and nursery schools to submit online. 

We would appreciate your assistance with contacting day cares about the due
date change for the annual immunization survey. Day cares centers and group
family day cares interested in submitting online, can send an email to
osas at health.state.ny.us or call 518-474-1944. Thank you for your assistance.
