Just an FYI, If you haven't already, you will be getting this notice from the NYS Dept of Health....

SURVEY IS DUE MARCH 5, 2015

Dear Day Care/Pre-k Administrator:

The New York State Department of Health, Bureau of Immunization Online Day Care and Pre-k Immunization Survey is ready for day cares and pre-ks to use. It is mandatory for all day cares and pre-ks to report the immunization survey data online by March 5, 2015.

Important Note:
Public Health Law (PHL), Rules and Regulations Subpart 66-1 addressing day care/pre-k immunization requirements has been amended. The amendment became effective on July 1, 2014. Detailed information regarding the revised immunization requirements which includes the Immunization Requirements for School Entrance/Attendance 2014-15 chart is available at http://www.health.ny.gov/prevention/immunization.schools/updated_school_imm_requirements.htm.

Each day care and pre-k must have a Health Commerce System (HCS) account in order to access the survey. If your school has not yet received an HCS account, please send an email to osas at health.ny.gov<mailto:osas at health.ny.gov> for information on how to apply for an account.

Additional resources are available to aid you in completing the survey:
* Online Day Care and Pre-k Immunization Survey Quick Guide, School Survey Instruction Booklet and Immunization Worksheets available at http://www.health.ny.gov/prevention/immunization/schools,
* Webinar training available at https://meetny.webex.com (registration instructions posted at: http://www.health.ny.gov/prevention/immunization/schools).

The immunization survey must be completed by March 5, 2015. Failure to complete and submit the survey form is a violation of Public Health Law, which may result in the imposition of a civil penalty.

Please contact the NYSDOH, Bureau of Immunization, School Assessment Unit and Compliance at 518-474-1944 or send an email to osas at health.ny.gov<mailto:osas at health.ny.gov> if you have any questions. Thank you for your assistance in monitoring immunization information and helping to ensure high immunization rates among the children of New York State.
