NYSDOH Annual Immunization Survey Due Date Change! - Child Care Coordinating Council & Child Care Councils<o:p></o:p></span></p></div></div><p class=MsoNormal><o:p> </o:p></p><p><span style='font-size:10.0pt'>Dear Child Care Coordinating Council and Child Care Council Representatives:</span><br><br><b><span style='font-size:10.0pt'>We are moving the day care/pre-k survey to the beginning of February with a March 5 due date. </span></b><br><span style='font-size:10.0pt'><br>The October 31, 2011 survey due date will apply only to all public and non-public schools (Pre-k/K through 12).</span><br><span style='font-size:10.0pt'><br><b>The March 5, 2012 survey due date will apply to all day care centers, Head Start Programs, nursery schools and group family day cares.</b></span><br><span style='font-size:10.0pt'><br>We will be creating a separate online day care survey and are actively contacting day care centers, Head Start programs and nursery schools regarding submitting online. </span><br><span style='font-size:10.0pt'>Group family day cares are welcome to submit online, but we are not actively contacting them at this time. We anticipate making it mandatory for day care centers, Head Start programs and nursery schools to submit online. </span><br><br><span style='font-size:10.0pt'>We would appreciate your assistance with contacting day cares about the due date change for the annual immunization survey.
