<p class="MsoNormal" style="text-indent:.5in"><span style="font-size:12.0pt;font-family:"Times New Roman",serif">The New York State Department of Health, Bureau of Immunization Online Day Care and Pre-k Immunization Survey is ready for day cares and pre-ks
to use. <b>It is mandatory for all day cares and pre-ks to report the immunization survey data online by March 5, 2015.<o:p></o:p></b></span></p>
<p class="MsoNormal" style="text-indent:.5in"><b><span style="font-size:12.0pt;font-family:"Times New Roman",serif"><o:p> </o:p></span></b></p>
<p class="MsoNormal" style="text-indent:.5in"><b><u><span style="font-size:12.0pt;font-family:"Times New Roman",serif">Important Note:</span></u></b><span style="font-size:12.0pt;font-family:"Times New Roman",serif"><o:p></o:p></span></p>
<p class="MsoNormal" style="text-indent:.5in"><span style="font-size:12.0pt;font-family:"Times New Roman",serif">Public Health Law (PHL), Rules and Regulations Subpart 66-1 addressing day care/pre-k immunization requirements has been amended. The amendment
